If you are interested in becoming a new patient in either Arlington or Plymouth, please call our Arlington Office at 781.641.1901  and follow the prompts. We make every effort to reach new patients as soon as possible, and pledge to return your call within 7 business days.

Once you schedule your first appointment, below is a checklist to help prepare for your visit:

• Download all of your New Patient Forms. Please bring all forms completed and signed for your first appointment. If you are unable to print them out, please arrive 15 minutes early to fill them out with us.
• Confirm with your insurance company regarding coverage for your appointment.
• If you are coming for a non-covered service, please bring payment in the form of Visa, Mastercard, American Express, Check or Cash. Payment is expected at the time service is rendered.
